MedSoc Feedback/Reporting Form

This is MedSoc’s official form to give us feedback and to let us know what we could do better. Additionally you can use this form to report any incidences which you would like to inform MedSoc about or use this tool to tell us things you want to be passed onto the medical school.

You can submit a response either anonymously or include details about yourself, however we can only get in contact with you to let you know the progress of what has been done regarding your submission if you do include the necessary details, additionally we may not be able to fully act on an anonymous report depending on the circumstances.

Your report will be sent to the welfare rep, and handled accordingly by the fewest members of MedSoc committee relevant to the issue (e.g. inclusivity reps, diversity reps etc.) but if there are any objections to certain committee members hearing of the report that you have please state these at the end of the form.

Please remember this is a tool made by students for students, misuse of the tool will result in it’s loss of use for all students or the cessation of anonymous reporting so please refrain from misuse.
